Parkinson's Database Survey Part 3 - Medical

This multi part Survey contains many questions relating to your general demographics, lifestyle and health. It is completely anonymous and the question responses are randomized. This means that even we are unable to determine which participant answered which questions. Your IP address, URL and web server are not tracked in this survey. 

Follow up questions will be based on initial responses and continue as commonalities are seen. These updates should occur monthly, but perhaps more frequently depending on how quickly surveys are completed. Updates will serve to establish more detailed picture of possible contributors to this disease narrowing down the field of possible causes.

There will be no e-mail notifications on this multi part survey as registration is not required. Please check our survey page and only complete each section once.

Please take your time and answer as many questions as you are able. Most importantly, answer honestly and as completely as you are able.

Remember that the only one who will know what your responses to the questions are is you.

In keeping this survey anonymous it protects both the patients and the researchers. It also will help to give us an unbiased look at all aspects of this disease.

The record of your survey responses does not contain any identifying information about you, unless a specific survey question explicitly asked for it. If you used an identifying token to access this survey, please rest assured that this token will not be stored together with your responses. It is managed in a separate database and will only be updated to indicate whether you did (or did not) complete this survey. There is no way of matching identification tokens with survey responses.
